The President’s Emergency Plan for AIDS Relief (PEPFAR) is the largest commitment by any nation to fight a single disease. Since 2003, it has saved over 26 million lives, prevented 5.5 million babies from being born with HIV, and supports 20.5 million people with treatment across 55 countries. It is a globally respected symbol of American leadership.

On January 24, 2025, a 90-day suspension of foreign aid was ordered, disrupting the PEPFAR Program, which provides lifesaving support to 20.5 million people living with HIV across 55 countries. The suspension immediately jeopardized access to daily HIV medication for 222,000 people, the administration of 224,000 daily HIV tests, essential services for vulnerable populations, and the work of 190,000 healthcare workers engaged in the global HIV/AIDS response. 

Although a waiver was issued to resume some critical services, most USAID-supported PEPFAR programs lost funding on February 26, 2025 — making what was intended as a temporary freeze likely permanent without urgent policy intervention.

Already, an estimated 35,839 people have died due to the partial PEPFAR shutdown. The UN estimates 6 million lives will be lost over the next four years if this program is shut down entirely.

In response, the Alliance for American Leadership is scheduling over 150 meetings with members of Congress and their legislative assistants in the coming month to advocate for the restoration of PEPFAR funding. Without action, the stakes are catastrophic: according to the United Nations, up to 6 million people could die over the next four years if PEPFAR is fully shut down.
